Home health agencies offer a variety of services for patients with a variety of conditions. Eligibility requirements and qualifications can sometimes be overwhelming and often seem to be constantly changing. Educational Objectives:
- Recognize the services provided by certified home health agencies, how the programs function, patient insurance coverages, and the initial and ongoing roles of referring/ordering practitioners.
- Identify the range of conditions and patients across the continuum that are served by certified home health agencies, and for which referral for services may be indicated.
- Summarize the criteria, documentation, and processes for patient referral to certified home health agency services.
- Examine the requirements for eligibility/qualification for referral, review/signature of orders and face-to-face certifications, calls from home health professionals.
