Support your patients in achieving their recovery, housing, employment, and social goals with personalized peer guidance. Their statewide service connects eligible individuals to the Office of Mental Health’s Community Oriented Recovery and Empowerment (CORE) services.
- Individuals aged 21 and over
- Insured through Managed Medicaid
- Frequent users of inpatient, emergency room, or detox services for mental health or substance use
A simple phone call to their toll-free 1-855-PEER NAV (855-733-7628) Navigator Line, with or without a referring provider, links patients to a Peer Navigator in your community.
